By proceeding with this service, I confirm that:

  I understand that this is a screening tool, not a medical consultation or diagnosis.

  I understand that CSA does not provide clinical advice. I should consult a GP or health professional if I have concerns.

  I consent to CSA collecting, storing, and using my data in accordance with its Privacy Policy.

  I consent to the use of my de-identified data for service quality improvement, health trend analysis, and ethically approved research purposes. My identity will not be disclosed or identifiable from this data.

I understand that I may withdraw my consent at any time by contacting CSA. From the time my consent is withdrawn:

  No further personal or identifiable data will be collected or used by CSA

• 

  My existing de-identified data may continue to be used for public health research, service evaluation, and quality improvement activities, in line with ethical and legal standards.

• 

  This de-identified data will not be linked back to me in any way, and it will not be possible for me to be identified from its use.
  

This service has been developed in alignment with Australian ethical guidelines for non-medical wellness and risk screening services. Any elevated risk identified will prompt a recommendation that I seek further advice from a health professional. All staff are trained to ensure respect, dignity, and confidentiality.
